Research using mHealth apps has the potential to positively impact health care management and outcomes. However, choosing an appropriate mHealth app may be challenging for the health researcher. The author team used existing evaluation tools, checklists, and guidelines to assess selected mHealth apps to identify strengths, challenges, and potential gaps within existing evaluation tools. They identified specific evaluation tool components, questions, and items most effective in examining app content, usability, and features, including literacy demand and cultural appropriateness; technical information; practical (...) aspects of app functionality; and evolving capabilities of mobile medical apps. Challenges included the subjective nature of the results, time required to complete the evaluation, lack of emphasis on evidence‐based content, and inadequate tool flexibility. Health researchers considering the integration of mobile apps into research will benefit from evaluation tools that assess both evidence‐based content and the ability of the mobile app to securely integrate with other digital technologies involved in patient care. Next steps will include the involvement of health care providers and professionals, including nurses a wide range of expertise, to develop an mHealth evaluation tool that focuses on identifying quality, evidence‐based mobile apps into patient outcomes research. Under current laws, adolescents caught sexting are being arrested, facing child pornography charges, and having to register as sex offenders. State laws on child pornography and child abuse differ throughout the United States and conflict with federal laws, making the ethical obligations for psychologists unclear. Nanomedicine is yielding new and improved treatments and diagnostics for a range of diseases and disorders. Nanomedicine applications incorporate materials and components with nanoscale dimensions where novel physiochemical properties emerge as a result of size-dependent phenomena and high surface-to-mass ratio. Nanotherapeutics and in vivo nanodiagnostics are a subset of nanomedicine products that enter the human body. These include drugs, biological products, implantable medical devices, and combination products that are designed to function in the body in ways unachievable at larger scales. (...) Nanotherapeutics andin vivonanodiagnostics incorporate materials that are engineered at the nanoscale to express novel properties that are medicinally useful. These nanomedicine applications can also contain nanomaterials that are biologically active, producing interactions that depend on biological triggers. ABSTRACT Although Kairos in Greek mythology is often depicted as the winged son of Zeus who grants to those who lay hold of his single lock of hair their once-in-a-lifetime opportunity, in traditional African American culture, particularly when it comes to speech, Kairos is essentially family. Given how much African American speakers depend on seizing the moment to invoke spiritual connections, emit laughter, and profess the truth, Kairos, or what we might call CPT (“Colored People’s Time”), can be summoned almost (...) at will. One of the African American discourses this article will use to illustrate this point is Call and Response, a verbal exchange in which speakers and listeners attune to one another and to the timeliness of an event. Martin Luther King Jr.’s “I Have a Dream” speech was such an event, but it wouldn’t have been so were it not for the timely responding of his favorite gospel singer, Mahalia Jackson. (shrink)

In February 2014, the Belgian Parliament passed legislation allowing euthanasia for terminally ill children of all ages by 86 votes to 44, with 12 abstentions. The Bill became law in early March after being signed by the King, making Belgium the first country in the world to abolish age restrictions for euthanasia. Previously, the youngest age at which euthanasia was permitted was 12 years old in The Netherlands.1Euthanasia was legalised in Belgium in 2002, and the new legislation introduces amendments (...) to the law extending euthanasia to minors in certain circumstances. Euthanasia is now permissible for children of all ages where the child has a “terminal and incurable illness”, experiences “constant and unbearable physical suffering”, and death is expected to occur within a “brief period”. The child must be deemed capable of making the decision, and have the agreement of their parents.Belgian law sets no timetable for euthanasia from the point at which the patient first expresses a wish to die. As with adults seeking euthanasia, any request must be made in writing. A physician and “outsider” brought in to give a second opinion must agree upon the diagnosis and prognosis, and a paediatric psychiatrist or psychologist must certify in writing that the child possesses “the capacity of discernment”. Following this, the child's physician must meet with the parents to inform them of the outcome of the consultation and ensure they are in agreement with the child's request. What kinds of time are opened, disclosed, or foreclosed in such moments? This section explores a concept that has a long history in rhetoric and philosophy, but which is urgently called for now, in a time that many think of as critical, catastrophic, or even apocalyptic. Changes in the economy, (...) climate, and the state of our democracies demand urgent attention, and while people disagree on the course to be taken, there is a sense that—this is it!—now is the time. The concept of kairos (from ancient Greek καιρός) comprises both a critical time and a perfect opportunity; it is the right moment to act, even though the word could also be interpreted in a more general sense as referring to the issue of right timing. Considered as "one of the most untranslatable of Greek words," kairos is perhaps related to the verb kurō, "to meet" or "meet accidentally," as when an arrow meets a target, suggesting that there is a spatial component in the temporal kairos.1 The spatial dimension shines through in the earliest uses of the term discussed in both SeungJung Kim's article on ancient Greek visual arts and Robert Sullivan's article on Isocrates (436–338 BCE). According to Sullivan's survey, Isocrates most often employs the word to refer to a specific situation, occasion, state of affairs, or set of circumstances.How do you recognize, let alone seize, this kind of moment, though? The best-known depiction of this difficulty is a portrait of Kairos personified that dates back to Lysippos in the fourth century BCE, reconstructed visually in three dimensions in Kim's essay. In Greek mythology Kairos is the god of golden opportunities, which (as we all know!) tend to pass by too quickly. The portrait shows a winged figure with a flowing forelock that ideally gives you something to hold on to. This paper reconsiders semiotic modelling in light of recent scholarship on Charles Peirce, particularly regarding his concept of proposition. Conceived in the vein of Peirce’s phenomenological categories as well as of his taxonomy of signs, semiotic modelling has mostly been thought of as ascending from simple, basic sign types to complex ones. This constitutes the backbone of most currently accepted semiotic modelling theories and entails the further acceptance of an unexamined a priori coherence between complexity of cognition and complexity of (...) signification. Following recent readings of Peirce’s post-1900 semiotic, we engage in a discussion as to what are the limits of this approach. From Stjernfelt’s conception of the dicisign in nature, we derive a perspective that affords understanding the practice of modelling as a reciprocal interplay between decomposition of complexity and recombination into further complexity. This discussion is facilitated by the recent extrapolation of the constructivist concept of scaffolding in biosemiotics research. Cognition, we argue, begins with a fundamental irritation of trying to make sense of a structure that is more complex than what can directly be derived from experience and, in so doing, urges meaning-seeking processes. The yet unknown object is decomposed into more tangible objects and is subsequently reassembled from these more manageable conceptions of the object. In support of our argument, we discuss the notions of semiotic competences and resources in light of such a naturalized account of meaning-making. (shrink)

Definition of the problemThe article addresses the relationship between reproduction, time and the good life. Services offered by reproductive medicine and conceptions of the good life in time influence each other reciprocally. This interaction is characterised by implicit and explicit normative settings and expectations of appropriate temporality.ArgumentsWe first discuss the significance of time for the life course and for parenthood from a sociological and social psychological perspective. Reproductive medicine can increase the options for becoming a parent and thus for life-time (...) autonomy. However, in the context of societal imperatives for optimisation and efficiency, it can also have opposite effects and increase heteronomy. Overall, this leads to changed forms of adaptation and self-determination, new dilemmas and ambivalences in the temporal optimisation of parenthood.ConclusionsAgainst this background, we explain how the ethics of reproductive medicine could benefit from interdisciplinary research that focuses on life in its temporal course. Avon Longitudinal Study of Parents and Children is a birth cohort study within which the Project to Enhance ALSPAC through Record Linkage was established to enrich the ALSPAC resource through linkage between ALSPAC participants and routine sources of health and social data. PEARL incorporated qualitative research to seek the views of young people about data linkage, including their opinions about appropriate safeguards and research governance. In this paper we focus on views expressed about the purpose and composition of research ethics (...) committees. Digitally recorded interviews were conducted with 48 participants aged 17–19 years. Participants were asked about whether medical research should be monitored and controlled, their knowledge of research ethics committees, who should sit on these committees and what their role should be. Interview recordings were fully transcribed and anonymised. Thematic analysis was undertaken, assisted by the Framework approach to data management. The majority of interviewees had little or no specific knowledge of ethics committees. Once given basic information about research ethics committees, only three respondents suggested there was no need for such bodies to scrutinise research. The key tasks of ethics committees were identified as monitoring the research process and protecting research participants. The difficulty of balancing the potential to inhibit research against the need to protect research participants was acknowledged. The importance of relevant research and professional expertise was identified but it was also considered important to represent wider public opinion, and to counter the bias potentially associated with self-selection possibly through a selection process similar to ‘jury duty’. There is a need for more education and public awareness about the role and composition of research ethics committees. Despite an initial lack of knowledge, interviewees were able to contribute their ideas and balance the rights of individuals with the wider benefits from research. The suggestion that public opinion should be represented through random selection similar to jury duty may be worth pursuing in the light of the need to ensure diversity of opinion and establish trust amongst the general public about the use of ‘big data’ for the wider public good. Vern Bullough and Cameron Campbell's study of the medieval diet and female longevity concluded that early-medieval women suffered from iron deficiencies triggered jointly by poor nutrition and frequent childbearing and that these deficiencies contributed substantially to their average early age of death. Ann (...) Hagen's overview of Anglo-Saxon patterns of food production and consumption suggested that most of the early English population routinely lived at marginally adequate or outright substandard levels of nutrition. Similar conclusions were reached by Renée Doehaerd in her study of the early-medieval economy. Michel Rouche, on the other hand, asserted that the typical Carolingian—including the peasants—had access to a monotonous, but abundant, supply of foodstuffs and may have consumed an average of 6,000–9,000 calories per day. Richard Hodges likewise decided that Anglo-Saxon peasants were reasonably well fed, based on the heavy food rents per hide demanded during the reign of the West Saxon king Ina. (shrink)

The human major histocompatibility complex (MHC), on the short arm of chromosome 6, represents one of the most extensively characterised regions of the human genome. This ∼4 Mb segment of DNA contains genes encoding the polymorphic MHC class I and class II molecules which are involved in antigen presentation during an immune response. Recently the whole of the MHC has been cloned in cosmids and/or yeast artificial chromosomes (YACs) and large portions have been characterised for the presence of novel genes. (...) Many unrelated genes, both housekeeping and tissue specific, have been identified and the gene density in some regions is now approaching one gene every few kilobases. Some of the novel genes encode proteins involved in the intracellular processing and transport of antigens that are presented by MHC class I molecules. Others, however, have no obvious role in the immune response. The MHC is located in the chromosome band 6p21.3 which is a Giemsa (G)‐light band. The detection of such a large number of functional genes (at least 70) in this region is compatible with the idea that both housekeeping and tissue‐specific genes are localised predominantly in G‐light bands. Thomas Piketty’s Capital in the Twenty-First Century makes hardly any reference to the ethics of inequality. Surprisingly, this is an omission shared by most of his critics. In this paper I investigate the literature on which he and his reviewers might have drawn and speculate on the reasons why they did not. I outline the four ‘views of society’ and the related issues in moral philosophy that were presented by Michael Schneider in his book on the distribution of wealth. I (...) then summarise the criticisms of Piketty made by those few reviewers who did show some interest in ethical questions and examine the slightly earlier and quite different case against reducing inequality made by one of these critics, N. Gregory Mankiw. I consider the economic, political and social costs of inequality identified in a book-length study of Piketty’s work by Steven Pressman, and conclude by reflecting on the reasons for the widespread neglect of moral philosophy by mainstream economists. Complete moral consensus on the status of the human embryo is neither feasible nor necessary for the formulation of ethically acceptable public policy for human embryo research. Significant consensus on permissible human embryo research can rest upon diverse but overlapping moral traditions. Thus, human embryo research policy should do more than reflect mere abstract assertions about the moral status of human embryos. Rather, the moral underpinnings of human embryo research should be derived from a range of values, including the facilitation (...) of human procreation, the advancement of applied scientific knowledge, the reduction of human suffering, and the protection of vulnerable persons from coercion and exploitation. The concept of structure is central to Giddens’s structuration theory because it apparently accounts for the reproduction of the social system without derogating the lay actor in functionalist or structuralist fashion. In fact, the concept of structure involves the very derogation of the lay actor which Giddens highlights as the principal error of these objectivist social theories and which he wishes to avoid. However, although Giddens fails to recognize it, the concept of “practical consciousness” which Giddens also regards as central (...) to structuration theory avoids this relapse into objectivism and offers a route to a sustainable social theory which highlights the agency of social actors. This article reviews the effects of malnutrition on early brain development using data generated from animal experiments and human clinical studies. Three related processes, each with their own functional consequences, are implicated in the alteration of brain development. (1) Maternal undernutrition at the start of pregnancy results in reduced transfer of nutrients across the placenta, allowing the conservation of effort for future reproductive episodes. (2) Differential allocation to growing organs by the fetus in response to nutritional stress spares the brain (...) to a large though still limited degree, reflecting the organ’s relative contribution to survival and reproductive success. (3) Prenatal malnutrition disrupts developing neurotransmitter systems, which results in the expression of specific cognitive and affective traits. It is argued that the increasing size and therefore cost of the brain, in conjunction with increasing ecological instability and marginality, reinforced selection for maternally controlled growth suppression of offspring, reallocation of organ growth rates by offspring, and behavioral changes related to development of neurotransmitter systems. This study explores African students' use of cultural metaphoric reasoning in classifying everyday situations as hot or cold, as is part of Sotho cultural tradition. It documents the extent to which such metaphoric reasoning is related to the use of science (mis)conceptions of heating. Written probes were used to document cultural metaphoric reasoning and science misconceptions of students entering a university science program. The same instruments were used as postprobes after a 4-week teaching intervention using experimental cognitive conflict strategies for (...) various standard misconceptions on heating. The findings show that almost a third of the sample used metaphoric reasoning consistently. This group was, as expected, significantly dominated by Sotho speakers. Initially, a large percentage of students explained the particulate effect of heating in terms of melting, shrinking, or breaking up of atoms. Most students identified specific materials as good insulators against coldness or against heat. Heat transfer was frequently explained in terms of absorption, diffusion, or reflection rather than conduction. There were no significant differences in the type and frequencies of these alternative concepts held by cultural metaphoric and noncultural metaphoric reasoners before the intervention. The teaching program did not affect cultural metaphoric reasoning, but had significant and equal benefits for the conceptual understanding of both groups, with the exception that the cultural metaphoric reasoners were more persistent in attributing observable properties to atoms. The present study suggests some further modification to the teaching strategies and concludes that, at this level of education, students are able to distinguish between different types of reasoning appropriate for cultural everyday situations and scientific contexts. W. E. Hocking has written recently that Whitehead's descriptive generalization of concrete fact, namely, his actual occasion, is “… not a term of description in the direct sense. It is an hypothesis. It cannot be kept in place by pointing to its presence as a datum: it can only hold its own if it proves to be a valuable conceptual tool.” I further advance the thesis that all generality is hypothetical, and holds it own only if it proves to be (...) conceptually useful. This is in accord with Whitehead's statement, the full implications of which have not been taken seriously enough, that metaphysical categories are not dogmatic statements of the obvious; they are tentative formulations of the ultimate generalities. But when these tentative formulations prove conceptually useful or are conceptually employed, they lose their “hypothetical air,” and often pass into what Whitehead calls a false state of “sober obviousness:”“An old established metaphysical system gains a false air of adequate precision from the fact that its words and phrases have passed into current literature. Thus propositions expressed in its language are more easily correlated to our flitting intuitions into metaphysical truth. When we trust these verbal statements and argue as though they adequately analysed meaning, we are led into difficulties which take the shape of negations of what in practice is presupposed. But when they are proposed as first principles they assume an unmerited air of sober obviousness.”. (shrink)

Since Schleiermacher and Hegel it has been part of the tradition of German philosophy to view the history of philosophy as an essential aspect of philosophy itself. The topic of “Heidegger and the History of Philosophy” should be considered with this in mind, which means that our question is as follows: Within this general attitude towards the history of philosophy, which has dominated German philosophy since Hegel, what distinctive attitudes are to be found in Heidegger? The context assumed by the (...) question is clear: it is that created by the rise of historical consciousness. It was part of the heritage of German Romanticism that not only historical research as a whole but also the position of philosophical theories would henceforth be affected by the problem of the nature of history. Before the Romantic period there was no such thing as the history of philosophy in the sense we are concerned with here. There was only the sort of scholarship which compiles lists. Hegel’s concept of a history of philosophy was itself in the fullest sense “philosophy;” it was a special subdivision of the philosophy of history, which was itself an attempt to demonstrate the presence of reason in history. Indeed, Hegel termed the history of philosophy the very heart of world history. However, the substantive portion of the Hegelian history of philosophy—its attempt to exhibit the necessity underlying the sequence of forms of philosophical thought, and thus the “rational” character of the history of thought—did not survive the criticism of the “historical school.”.
